Take control of your Care Plan. Before you come to write your Care Plan, ask yourself the following questions:

  • Is my medication right for me?
  • Are there other therapies that might help?
  • Can my physical health be improved?
  • Can my living arrangements be improved?
  • Do I need help with money issues?
  • Can my employment situation be improved?
  • Can my social life be improved?
  • Are there any education/training issues that need addressing?

Think about your short-term and long-term goals in these areas and then make sure they are recorded in your Care Plan. Next ensure that your Care Plan is reviewed regularly so that you can assess your progress towards your goals and even modify them, as well as setting new goals.
